Sean Forster, MSN, RN
Chief Executive Officer
Sean Forster has over ten years of experience in psychiatric services. Sean started his career as a Nursing Assistant in assisted living homes while he progressed through his schooling and obtained first is LPN and then his BSN-RN Degree. Sean worked as a behavioral health unit nurse and progressed into a coordinator, case manager, and Director of inpatient behavioral health services in North Dakota. Sean also oversaw outpatient mental health services from the clinic, PHP, and IOP programs before moving to Colorado and joining Centennial Peaks Hospital as their CNO in 2017. In 2019, Sean transitioned from CNO to the hospital CEO.

Patty Rivers, CPA
Chief Financial Officer
Patty Rivers joined Centennial Peaks Hospital in January 2020. Patty graduated from the University of Colorado-Boulder, the University of Nebraska, and the University of Alaska. She began her accounting and finance career with the international accounting firm KPMG, and also has experience as the CFO of a large hospital system in Alaska. She has 14 years of experience in behavioral health facilities in Colorado. She is licensed in Colorado as a Certified Public Accountant (CPA).

Elyse Marinelli, LCSW
Director of Risk Management and Performance Improvement
Elyse Marinelli received her Master of Social Work degree from Colorado State University and is an LCSW in the state of Colorado. She joined Centennial Peaks Hospital in 2017 as a social worker on the adolescent unit. She was promoted to Social Work Coordinator before transitioning into the role of Director of Risk Management and Performance Improvement in 2020. Elyse has 7 years of clinical experience in residential treatment and hospitals working with children, adolescents, adults, and families. She is trained in Trauma-Informed Care which enables her to assist in individualized treatment. This enables her to ensure that patients are on appropriate precautions during their treatment to maximize safety and ensure each patient has a therapeutic environment.

John Guenther
Director of Plant Operations
John Guenther began his career working within the treatment center units at Colorados juvenile detention centers for seven years as a youth counselor. In 2006, he joined the team at Centennial Peaks Hospital as a unit specialist. John spent the next two years growing and delivering clinical care to adolescents enrolled in the long-term care program before transitioning into mechanical contracting. In 2010, John became a maintenance technician and after a year was promoted to Director of Plant Operations. Since then, he has been promoted to Senior Director of Plant Operations providing support and oversight to Centennial Peaks two Colorado sister facilities, supporting operations and regulatory compliance at all three hospitals.

Erika Donnellan, MA, BCBA, SPHR, SHRM-SCP
Human Resources Director
Erika joined Centennial Peaks Hospital in 2018 as the Director of Human Resources. She received her Master’s degree in Industrial Organizational Psychology, with a focus on Organizational Behavior Management (OBM), at Western Michigan University. She began her career in Applied Behavioral Analysis; seeking to improve organizational effectiveness and quality care using the principles of Performance Management. Her career shifted into Human Resources over the years focusing on all phases of people management including full cycle talent acquisition, employee relations, performance management, retention, compensation, benefit programs, and coaching support to the leadership team. She is dually certified in Human Resources with a Senior Professional in Human Resources (SPHR) and Society for Human Resources- Senior Certified Professional (SHRM-SCP).

Anmarie Masters, M.Ed, RHIA, CPCS, CPMSM
Director of HIM
Anmarie Masters is the Director of Health Information Management Services for Centennial Peaks Hospital. She joined Centennial Peaks in 2014. She has 17 years of experience in the medical field. Anmarie earned her Bachelor of Science degree in Health Information Management from Regis University in 2005 and in 2009, she completed a master’s degree program from Colorado State University. Anmarie became a certified Registered Health Information Administrator in 2006 through the American Health Information Management Association.

April Zink, MS, LAC
Director of Utilization Review
April Zink is a Colorado native having been born in Greeley. She completed her undergraduate degree from UNC in Greeley and her Master’s degree from Walden University in 2007. She completed her Master’s Thesis while working with the Wing and Squadron Commander at McChord Air Force Base in a study of depression in military spouses of C-17 pilots. April began working at Centennial Peaks Hospital in 2013 as a social worker, then transitioned to being the Assistant Clinical Director. In 2017 she became Director of Utilization.

Nancy Burlack, Ed.D, LMFT
Director of Outpatient
Nancy Burlak earned her Doctorate Degree in Counseling Psychology in 2014, and also holds Master’s degrees in both Psychology and Criminal Counseling. Dr. Burlak is a licensed Marriage and Family Therapist in the states of Colorado and California and specializes in trauma treatment as a Certified Clinical Trauma Professional (CCTP). Along with her dedication to serving others, Dr. Burlak brings over 25 years of experience in the mental health field as both a clinician and leader across a variety of settings. Dr. Burlak joined Centennial Peaks in 2019 as the Director of Outpatient Services after working as a surveyor with the Joint Commission. Passionate about meeting the specific mental health needs of our community, Dr. Burlak has developed outpatient programming to include several specialty therapy groups offered in the Centennial Peaks Outpatient Department.

Danielle Worden
Director of Business Office
Danielle is originally from San Diego, California, and moved to Colorado in 2006. Danielle joined UHS in 2015 as a financial counselor and found her passion for behavioral health and hospital finance. Danielle has been a certified Paralegal since 2014 but changed her degree to Accounting through SNHU in 2017. Danielle joined Centennial Peaks as Business Office Director in 2020.

Laura Burnett-Tubbs, MA LPC
Director of Intake
Laura Burnett-Tubbs is the Director of Intake at Centennial Peaks Hospital. She has a Bachelor’s of Science in Neuropsychology from Ithaca College in upstate New York and a Masters in Contemplative Psychotherapy from Naropa University in Colorado. Laura has worked as a clinician in multiple ER’s and community crisis centers, as well as having my own private practice. She began working at Centennial Peaks in June 2019.

Robert Cameron, MSN, RN
Chief Nursing Officer
Rob Cameron earned his Bachelor of Science Degree in Nursing from the University of Colorado in 2009 and his Master of Science Degree in Nursing Leadership from Western Governors University in 2015. Rob brings to the team more than 30 years of experience in healthcare and 20 years of experience in nursing, including Emergency Medicine, Critical Care nursing, nursing education, Correctional Care as well as Behavioral Health nursing and leadership. Rob joined Centennial Peaks in 2020 as the Chief Nursing Officer.

Sara Haislet, LCSW
Director of Clinical Services
Sara Haislet earned her Master’s degree in Social Work from Portland State University in 2014 and is a Licensed Clinical Social Worker in Colorado. Sara brings to the team over 10 years of experience in the mental health field. She began her career in 2009 as a mental health tech in Oregon prior to earning her master’s degree and has since worked in community and inpatient mental health across the country specializing in trauma-informed crisis mental health services. Sara joined Centennial Peaks in 2019 as a Social Worker and was then promoted into the Social Work Coordinator position before joining the leadership team as the Director of Clinical Services.
